  • Telehealth offers ease of access to exceptional care from the comfort of your home, however, some conditions require an in-person evaluation.

    Services that cannot be treated by telehealth include sore throat, ear pain, vision changes, severe dizziness, orthopedic injuries, flu/COVID testing, pneumonia, recurrent urinary issues, and physical exams. Please visit your local urgent care for further evaluation.

    Please seek emergency care if you are experiencing chest pain, shortness of breath, or have sustained a head injury of any kind.

  • If The Cincy FNP recommends going to an urgent care or emergency department, you will only be charged a $25 administration fee.

    This would occur if it is determined that you need evaluation for illnesses such as strep throat, ear infections, chest pain, respiratory distress, or orthopedic evaluations.

  • We understand that personal matters may arise unexpectedly, however, we kindly ask that you cancel your appointment at least 24 hours in advance. Cancellations within 24 hours of appointment time will be subject to a $25 cancellation fee.

  • The Cincy FNP only accepts patients 18 years of age and older.

  • At this time, we are an affordable cash-based telehealth clinic and do not accept health insurance.

  • To ensure that your visit is maximized, we encourage you to be prepared with the following items.

    • Complete all intake forms prior to visit.

    • Medical records and recent lab results.

    • Full medication list or photos of your prescription bottle(s) if prescribed from a provider other than The Cincy FNP.

    • Blood sugar readings from the last 3 days (if diabetic).

    • Blood pressure reading no sooner than 3 days before your visit.

  • To ensure that your visit is maximized, we encourage you to be prepared with the following items.

    • Complete all intake forms prior to visit.

    • Temperature reading no sooner than 12 hours prior to visit.

    • Blood pressure reading no sooner than 12 hours prior to visit.

    • Full list of current medications.

    • Complete medical, social, and surgical history.
